Description
Job Opportunity: RN Home Health (Travel)
Location: Newton, Massachusetts
Contract Length: 13 weeks
Job Summary
As a Home Health RN, you will:
- Assess patient conditions
- Implement care plans
- Provide ongoing education and support to patients and their families
- Perform admission processes and case management responsibilities
- Deliver patient care in accordance with community health practice standards and organizational policies
- Utilize thorough and timely electronic documentation and interdisciplinary communication
- Participate in weekend rotations approximately once every 6 weeks
- Travel to assigned cases as needed
Required Experience
- Minimum 1 year Certified Home Care experience
- Minimum 1 year Case Management experience
Required Qualifications
- Current RN license valid in Massachusetts
- Ability to travel to patient cases as assigned
- Weekend availability (rotations once every 6 weeks)

Required Credentials and Compliance
- Professional license verification (MA RN license; no conditional licenses accepted)
- Motor Vehicle Record (MVR) check within 45 days of start
- Criminal background checks including Massachusetts CORI, SSN trace, national sex offender registry, and exclusion lists
- Documentation of immunizations and health screenings:
- MMR (measles, mumps, rubella) vaccination or immunity proof
- Influenza vaccine (required during flu season)
- TB screening (skin test or blood test within 3 months of start)
- Hepatitis B vaccination or immunity (strongly preferred)
- Tdap vaccine (preferred)
- N95 respirator fit test within 365 days of start, updated annually
- Respirator Medical Clearance attestation
- Physician health statement within 365 days of start
- Documentation of two professional references within 4 years, including at least one supervisor or higher
- Resume including past 7 years of work experience with city and state
- Skills checklist completed within 1 year of submission
- Confirmation of assignment form upon acceptance and for extensions
- Education verification (highest level achieved)
Additional Notes
- Must submit a professional color photo (no selfies, no filters, plain background)
- No religious or medical exemptions allowed for N95 fit testing and influenza vaccine
- TB positive results require documentation of a negative chest x-ray
- All documentation must be current and updated as required for breaks in service or expiration
This position is ideal for RNs with home health and case management experience who are comfortable traveling within the assigned area and managing patient care in a community setting.
