Description
Job Opportunity: RN Home Health (Travel)
Location: Newton, Massachusetts
Contract Length: 13 weeks
Job Summary
As a Home Health RN, you will:
- Assess patient conditions
- Implement care plans
- Provide ongoing education and support to patients and their families
- Perform admission processes and case management responsibilities
- Deliver patient care in accordance with organizational philosophy, policy, and community health practice standards
- Utilize thorough and timely electronic documentation and interdisciplinary communication
- Participate in weekend rotations approximately once every 6 weeks
- Travel to assigned cases as required
Required Experience
- Minimum 1 year Certified Home Care experience
- Minimum 1 year Case Management experience
Required Qualifications
- Valid RN license for Massachusetts (NURSYS and Board of Nursing verification required; MA Conditional Licenses not accepted)
- Documentation of TB screening within 3 months of start date (skin test or blood test) and updated if there is a break >6 months
- Physician statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break
- N95 fit test within 365 days of start date, updated annually (no onsite fit testing; must use specific form)
- Influenza vaccine required during flu season (Oct 1 - Mar 15), no exemptions allowed
- MMR vaccination or proof of immunity required
- MVR check within 45 days of start date
- Criminal background checks including Massachusetts CORI, SSN trace, National Sex Offender Registry, and exclusion checks (OIG, GSA, Medicaid lists) within 45 days of start date and updated as required
- Documentation of two references within 4 years, including at least one supervisor or higher in skillset
- Resume including past 7 years of work experience with city and state
- Skills checklist completed within 1 year of submission
- Confirmation of assignment form required for all initial assignments and extensions
- Hepatitis B vaccination or immunity proof strongly preferred
- Tdap vaccine within 10 years preferred
- TB chest x-ray documentation if positive TB test
Additional Information
- Must submit a professional color photo (no selfies, no filters, plain background, no eyeglasses)
- Must be able to travel to patient cases as assigned
- Weekend rotations occur approximately once every 6 weeks
