Job Opportunity: RN Home Health (Travel)
Location: Newton, Massachusetts
Contract Length: 13 weeks
Job Summary
Provide clinical services to patients of all ages in their homes. Responsibilities include assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and families. Perform admissions and case management in accordance with organizational policies, using electronic documentation and interdisciplinary communication.
Required Qualifications
- Specialty: Registered Nurse (RN) - Home Health
- Experience:
- Minimum 1 year Certified Home Care experience
- Minimum 1 year Case Management experience
- Licensure: Current RN license in the state of Massachusetts
- Other Requirements:
- Reliable, independent transportation for home visits
- Willingness to work weekends, holidays, and be on-call as needed (weekend rotations once every 6 weeks)

Key Responsibilities
- Provide clinical care in patients' homes, ranging from newborns to elderly patients
- Assess patient conditions and implement individualized care plans
- Educate and support patients and families
- Manage admissions and case management duties per policy standards
- Maintain thorough and timely electronic documentation
Compliance and Health Requirements
- N95 fit test within 365 days of start date, updated annually
- Documentation of MMR vaccination or immunity
- Influenza vaccine required during flu season (no exemptions)
- TB screening within 3 months of start date (skin test or blood test), updated if >6 month break
- Hepatitis B vaccination or immunity preferred
- Physician health statement within 365 days of start date
- Documentation of two measles, two mumps, and one rubella vaccine or two MMR vaccines, or proof of immunity
- Documentation of recent TB chest x-ray if positive TB test
- Documentation of Tdap vaccine within 10 years preferred
- Criminal background checks, including CORI and national sex offender registry
- Exclusion checks against OIG, GSA, MassHealth, and NH Medicaid lists
- Respirator Medical Clearance attestation
- Resume including 7 years of work experience with city and state
- Two professional references within 4 years, including at least one supervisor or higher
Additional Notes
- Must be willing to work in a travel assignment capacity
- Weekend rotations occur approximately once every 6 weeks
- Must adhere to all organizational policies and credentialing requirements prior to start
