Job Opportunity: Case Manager (Registered Nurse)
Location: San Francisco, California
Employment Type: Travel/Contract
Contract Length: 15 weeks
Job Description
- Responsible for care coordination and care transitions planning throughout the acute care patient experience.
- Collaborates with physicians, utilization managers, medical social workers, and bedside RNs to ensure timely patient progression and appropriate level of care.
- Aims to prevent unnecessary admissions or readmissions by facilitating communication and resource coordination across the care continuum.
- Works with patients, families, interdisciplinary teams, nursing management, quality and ancillary services, third-party payers, review agencies, claims and finance departments, medical directors, contracted providers, and community resources.
- If assigned to the Emergency Department, addresses complex clinical and social situations efficiently to avoid unnecessary admissions.
Shift Details
- 4 shifts per week, 10 hours each (08:00 - 18:30)
- Rotating weekends
- No on-call required
Required Qualifications
- Valid Registered Nurse (RN) license in California (copy of professional license required)
- Minimum 2 years of case management experience, including experience within the last year
- Utilization management experience working with attending physicians supporting level of care protocols
- Utilization review experience
- Advanced Cardiac Life Support (ACLS) certification issued by American Heart Association or American Red Cross
- Basic Life Support (BLS) certification issued by American Heart Association or American Red Cross
- Pediatric Advanced Life Support (PALS) certification issued by American Heart Association or American Red Cross
- Neonatal Resuscitation Procedure (NRP) certification
- Trauma Nursing Core Course (TNCC) certification
- Management of Assaultive Behavior (MAB) certification
- National Institutes of Health Stroke Scale (NIHSS) certification
- Crisis Prevention Institute (CPI) certification
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Additional miscellaneous certifications as required by position specifics
Background and Compliance Requirements
- 7-year background check required within 6 months of start, including social security trace, county searches, sex offender search, education verification, and FACIS Level 3 screening.
- Annual updates required for EPLS/GSA/SAM, HHS/OIG, and FACIS screenings.
- Education verification required; Bachelor of Science in Nursing preferred.
- Drug screening: Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids due to California law).
- Physical exam required within 1 year of start.
- Fit mask test required within 1 year of start and must be renewed annually (with exceptions).
- Immunizations and titers required: Tetanus/Diphtheria/Tdap, Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella, Flu seasonal vaccination during flu season. Religious and medical exemptions considered with proper documentation.
- TB screening questionnaire required within 90 days of start and annually thereafter; negative TB test or chest x-ray required.
- Respiratory safety clearance required prior to start and annually (with exceptions).
Documentation and Verification
- Must provide full 7 years of work history with no gaps; employment verification required.
- Unit-specific skills checklist required and must be renewed annually.
- References: 2 professional references required for clinical and allied positions within 3 years of hire.
- Various facility-specific documents and policies must be reviewed and signed prior to start, including standards of business conduct, code of conduct, vendor training, and others.
