Description

Registered Nurse - Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 17 weeks


Job Summary

Provide clinical nursing services to patients of all ages in their homes. Responsibilities include assessing patient conditions, implementing care plans, and offering ongoing education and support to patients and families. Perform admissions and case management duties following established policies and standards, with thorough electronic documentation and interdisciplinary communication.

Must be willing to work weekends, holidays, and be on call as needed. Weekend rotations occur approximately once every 6 weeks. Reliable, independent transportation is required to make home visits.


Required Qualifications

  • Valid RN license in Massachusetts
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Minimum 1 year of Certified Home Care experience within the last year
  • Experience in a Home Care setting
  • Minimum 1 year of Case Management experience
  • Reliable transportation for home visits
  • Willingness to work weekends, holidays, and on-call shifts per department rotation

Required Onboarding Credentials and Documentation

  • Professional license verification (primary source, within 30 days of start)
  • Recent color photo (plain background, no filters, taken within last 6 months)
  • Motor Vehicle Record check (within 45 days of start)
  • Tuberculosis screening (skin test or blood test within 3 months, updated if break >6 months)
  • Measles, Mumps, Rubella (MMR) vaccination or immunity proof
  • Physician health statement signed within last 12 months
  • Criminal background check (within 45 days, including SSN trace and CORI check for Massachusetts)
  • National sex offender registry check (within 30 days, updated annually)
  • Exclusion checks (OIG, GSA, Medicaid lists) within 30 days and annually
  • Resume including last 7 years of work experience with city and state
  • Two professional references within last 4 years (minimum one supervisor-level)
  • Skills checklist completed within 1 year
  • Confirmation of assignment form upon acceptance and for extensions
  • Education verification (highest level achieved)
  • N95 respirator fit test within 12 months, updated annually (no exemptions allowed)
  • Respirator medical clearance attestation (if fit test required)
  • Nurse Aide Registry check within 30 days (required by MA Department of Public Health for all employees)

Additional Information

  • Weekend rotations approximately every 6 weeks
  • Must be able to work independently in patient homes
  • Electronic documentation and interdisciplinary communication required
