Job Opportunity: Case Manager (Registered Nurse)
Location: Sacramento, California
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Summary
- Must have Home Health Case Manager experience.
- Responsible for overall patient management including direct care with a designated visit productivity standard.
- Plans, organizes, and directs patient care services for assigned caseload in compliance with current standards and regulations.
- Coordinates interdisciplinary team to implement patient care plans, manage visit utilization, and control expenses.
- May assist with agency-wide orientation, competency evaluations, and provide clinical consultation related to expertise.
Shift Details
- Day shift, Monday to Friday
- 8 hours per day (8:00 AM – 4:30 PM)
- 40 hours per week
Required Qualifications
- Valid Registered Nurse (RN) license in California.
- Minimum of 1 year of Case Management experience, including experience within the last year.
- Experience in a Home Health setting.
- Current Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
Background and Compliance Requirements
- 7-year background check required within 6 months of start, including social security trace with all aliases, county searches, sex offender search, education verification, and motor vehicle report (required for this position).
- FACIS Level 3 screening required within 2 weeks of start and monthly thereafter.
- Annual updates required for EPLS/GSA/SAM, HHS/OIG, and other sanction lists.
- Education verification required; Bachelor of Science in Nursing preferred.
- Negative 9-panel drug screen within 6 months of start (excluding marijuana due to California law).
- Full 7 years of work history with no gaps on employment application; employment verification required for up to 7 years.
- Two professional references required within 3 years for clinical positions.
Certifications and Competencies
- BLS (Basic Life Support) - AHA or ARC only.
- Additional certifications may be required based on position specifics (e.g., ACLS, PALS) but verify with recruiter.
- Medication and pharmacology competency exams as applicable.
- Unit-specific skills checklist required and must be renewed annually.
Health and Immunization Requirements
- Physical exam within 1 year of start (MD note or approved form acceptable).
- Fit mask test within 1 year, renewed annually (specific mask types required).
- Adult Tetanus/Diphtheria/Tdap vaccination within 10 years or lifetime TDAP with booster within 10 years.
- Negative TB test (QGold, TSPOT, or PPD) within 90 days of start; annual renewal required.
- Immunity or vaccination documentation required for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (series or declination)
- Varicella
- Seasonal flu vaccination required during flu season (Nov 1 – Apr 30) or signed declination with mask-wearing requirement.
- Drug testing: Negative 9-panel drug screen within 6 months, excluding marijuana.
Facility-Specific Documents and Training
- Candidate must review and sign facility-required documents prior to start, including:
- Job description
- Standards of business conduct
- Code of conduct
- Vendor training (2024 and 2025 versions depending on contract date)
- Overpayment refund policy
- TB screening questionnaire (signed by candidate and healthcare provider)
- Religious exemption forms required for facial hair (PAPR) and certain vaccine exemptions; approvals handled on a case-by-case basis.
Additional Notes
- Motor vehicle insurance required and must be effective for the duration of the assignment due to driving responsibilities.
- ICU and telemetry RNs at certain locations may be required to take EKG competency exams onsite after start.
- Pending background check attestations are generally not accepted; conditional clearances require facility approval.
- Color blindness must be documented by a licensed healthcare provider if applicable.
- Respiratory safety clearance required and must be renewed annually unless exempted by role.
