Day 5x8-Hour (07:00 - 15:30)
Mid 5x8-Hour (15:00 - 23:30)
Variable start times between 7am and 3:330pm; every other weekend, floating to various units as needed

CCM/ACM
Job Description: Case Manager required to be cross trained on all units to work in as needed, Acute Care experience required
We are seeking seasoned inpatient Case Manager RNs with:
Direct hospital-based discharge planning experience to the community (not UM-focused roles)
Strong inpatient and Emergency Department case management experience
Working knowledge of California insurance requirements and county regulations, particularly related to complex discharges
Experience functioning independently in an acute care hospital setting
