Description
Registered Nurse (RN) - Home Health
Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Job Summary
As a Home Health RN, you will:
- Assess patient conditions and implement care plans.
- Provide ongoing education and support to patients and their families.
- Perform admission processes and case management duties.
- Deliver patient care following community health practice standards and organizational policies.
- Document thoroughly and communicate effectively within an interdisciplinary team.
- Travel to assigned cases as needed.
- Participate in weekend rotations approximately once every 6 weeks.
Required Qualifications
- Valid RN license in Massachusetts
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Minimum 1 year of Home Health experience within the last year
- Minimum 1 year of Case Management experience
Required Documentation & Compliance (Must be completed prior to or at onboarding)
- Professional license/certification verification.
- Resume including past 7 years of work experience with city and state details.
- Two professional references within the last 4 years (at least one supervisor-level).
- Skills checklist completed within 1 year.
- Background checks including:
- Criminal background check (within 45 days prior to start).
- Massachusetts CORI check (within 30 days prior to start).
- National sex offender registry check (within 30 days prior to start).
- Exclusion checks against OIG, GSA, MassHealth, and NH Medicaid lists (within 30 days prior to start and annually).
- MVR (Motor Vehicle Record) check within 45 days prior to start.
- Health and immunization requirements:
- TB test or equivalent blood test within 3 months of start date (updated if break >6 months).
- MMR vaccination or proof of immunity.
- Physician health statement signed by MD/DO/NP/PA within 365 days of start (updated if break >6 months).
- Respirator medical clearance and N95 fit test within 1 year (no exemptions allowed).
- Nurse Aide Registry check within 30 days of start (required by MA Department of Public Health for all employees).
- Submission of a professional badge photo (color, recent, plain background, no glasses).
- Confirmation of assignment form upon acceptance and for any start date changes.
- Social Security Number trace as part of background check.
Shift
- Day shift: 08:00 AM – 04:30 PM
Additional Notes
- Positive TB skin test requires documentation of a negative chest X-ray after the positive test.
- No religious or medical exemptions allowed for N95 fit testing.
- Fit testing cannot be performed onsite; must be completed prior to start.
- Weekend rotations occur approximately once every 6 weeks.
This position is ideal for experienced RNs with certified home health and case management backgrounds who are comfortable traveling to patient homes and working independently within community health settings.
