Contract role in Battle Creek, Michigan
Shift: Monday–Friday, 8:00 am–4:30 pm
Provides direct nursing care to participants in a PACE home care setting
Works independently and may manage complex clinical situations
Includes clinic and home-based care, home visits, on-call needs, and travel between centers as needed

Valid RN license in Michigan
Associate of Science in Nursing / Associate Degree in Nursing
Home Health experience within the last year
Experience working with geriatric patients
Current BLS certification issued by the American Heart Association
Current valid Michigan driver’s license
Current valid vehicle insurance
Reliable transportation
Must be 18 years of age or older
Must be fully vaccinated, or willing to be fully vaccinated, against Influenza, MMR, Varicella, Tdap/Td, Covid-19, and Hepatitis B

Proficiency with Microsoft Word and Outlook
Strong written and verbal communication skills
Ability to maintain confidentiality
Ability to make independent decisions
Ability to plan, organize, develop, implement, and interpret goals, objectives, policies, and procedures
Ability to use time and resources efficiently and effectively
Ability to provide exceptional customer service

Triages emergent situations
Monitors acute and chronic medical conditions in coordination with Primary Care Providers
Administers treatments, procedures, medications, immunizations, and other care as prescribed
Performs wound care, assessments, lab draws, and other nursing tasks
Works beyond normal hours, including weekends and holidays as needed
May be exposed to infectious waste, diseases, hazardous chemicals, TB, AIDS, and Hepatitis B virus
Subject to callback during emergency conditions
Must be able to drive in all types of weather
Must be able to lift up to 20 pounds
