Required Qualifications

• Valid Physical Therapist license in California
• Physical Therapist experience within the last year
• BLS
• Experience in a Home Health setting

Responsibilities

• Prioritize start-of-care visits
• Supervise Physical Therapy Assistants
• Submit start-of-care reports by the next day
• Complete clinical notes and other documentation within 48 hours of the visit
• Maintain timely communication with the supervisor, intake, and office
• Be responsive during normal business hours

Territory

Contra Costa County, California, with expansion as needed

Training

• 3 full days of training, including 1 in-house day and 2 field days

Reimbursement and Support

• Mileage reimbursement: $0.65.5 per mile from first to last patient
• Parking and tolls reimbursed
• Company car option available with a decreased bill rate

Productivity Requirement

• 30 points per week

Visit Mix Notes

• Start of care: 2 points
• Recertification, resumption of care, OASIS discharge, evaluation: 1.5 points
• Revisit and non-OASIS discharge: 1 point
