Job Description

• Outpatient Speech-Language Pathologist position
• Adult outpatient experience required
• Preferred experience with language, cognition, dysphagia, brain injuries, apraxia of speech, voice disorders, Parkinson’s disease, and pediatrics

Required Qualifications

• Valid Speech Language Pathologist license in Michigan
• Minimum 1 year of Speech Language Pathologist experience within the last year
• Basic Life Support (BLS) certification (American Heart Association or RQI accepted)
• ASHA Certification
• Experience working with adult patients
• Experience in an outpatient setting

Required Documentation and Compliance

• Valid Driver’s License or State ID (must be current throughout assignment)
• Social Security Card
• E-Verify compliance
• Background check within 30 days of assignment start, including:

  • 7-year Social Security Number Trace
  • National Criminal History Search (7 years)
  • National Sex Offender Database Search
  • FACIS Level 3 Search
  • Education verification (highest level)
• EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
• High school diploma/GED verification (not applicable if RN)
• Physical exam within 12 months of start (must note latex sensitivity)
• N95 Fit Mask test within 12 months of start (annual update required)
• Tdap vaccine within 10 years or declination
• Negative TB test or chest x-ray per policy
• Immunity documentation or vaccination for Measles, Mumps, Rubella (MMR), Hepatitis B, Varicella
• 10-panel drug screen with THC within 30 days of start (no medical marijuana cards or dilute screens accepted)
• Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31)
• Completion of facility-specific documents and orientation modules prior to start
• Two professional references from previous employers
• Current resume with at least 7 years of work history
• Skills checklist and competency exams as applicable

Additional Notes

• Fingerprinting via Michigan LARA required only for certain locations (not specified for this role)
• Orientation safety manual acknowledgment required
• No weekend or floating shifts
• Modules and facility-specific paperwork completed onsite at orientation
