Required Qualifications

Valid Physical Therapist license in Oregon
1 year of Physical Therapist experience
Physical Therapist experience within the last year
BLS - Basic Life Support issued by American Heart Association or Heart and Stroke Foundation
Experience in a Home Health setting

Schedule and Work Expectations

Weekend work not required
On-call not required
No floating between like units
Block scheduling not available
May visit up to 6 patients per day
Standard productivity: 30 points
Traveler must have their own transportation
Occasional travel radius of roughly 60 miles per day

Location and Candidate Preferences

Local candidates accepted within a 50–75 mile radius

Compliance and Health Requirements

Primary series COVID vaccination plus one booster
Physical exam within 1 year of start
Fit mask required
Tdap within 10 years
Negative TB test or chest X-ray
Measles, mumps, rubella, varicella, and hepatitis B documentation required
Drug screen required
Flu vaccination or declination required during flu season
Respirator clearance required

Additional Notes

Free parking
Mileage reimbursed at IRS standard rate
Business casual attire; scrubs allowed but not required
Charting uses EPIC Remote Client for Home Health, Rover, and Oasis
